Now accepting Initial Damage Assessment Forms

On April 11, Governor Walz declared a state of emergency which designated 66 counties and tribes impacted by the spring snow melt and severe storm Wesley.  Any disaster related costs and damages incurred on or after March 12, 2019 may be eligible for assistance. 

Any public entity within Polk County claiming to have any disaster related costs and/or damages to its organization should complete and submit their Initial Damage Assessment forms to the Polk County Emergency Management Department.

When these forms are received they are then used to serve as a tool to be utilized by local officials and others in conducting local damage assessments of publicly owned property. These forms enable local officials to determine the severity and magnitude of the event.
