Passports
First time applicants must appear in person before a Passport Agent and submit a completed passport application. You must also present proof of U.S. citizenship. This consists of either a certified copy of a birth certificate in the U.S., or an original Naturalization Certificate.
You will need two passport photos.
An adult passport is valid for 10 years; a minor’s passport is valid for 5 years.
Age 18 and Over
Your passport will be valid for 10 years. You must provide:
- Two professional photographs taken specifically for a passport.
- A valid government issued picture ID.
- A certified copy of your birth record (to be sent to the Passport Agency and returned with your passport).
- Your Social Security number.
- Your parents' birth names, birth dates, birth places (State or Country). The applicant must sign in front of a passport agent.
Age 16 - 17
Your passport will be valid for 10 years. You must provide:
- Two professional photographs taken specifically for a passport.
- A valid government issued picture ID or a parents' ID is acceptable.
- A certified copy of your birth record (to be sent to the Passport Agency and returned with your passport).
- Your Social Security number.
- Your parents' birth names, birth dates, birth places (State or Country). The applicant must sign in front of a passport agent. One parent must sign in front of a passport agent. The parent that signs must provide a valid government issued picture ID.
Age 14 - 15
Your passport will be valid for 5 years. You must provide:
- Two professional photographs taken specifically for a passport.
- A certified copy of your birth record (to be sent to the Passport Agency and returned with your passport).
- Your Social Security number.
- Your parents' birth names, birth dates, birth places (State or Country). The applicant must sign in front of a passport agent. One parent must sign in front of a passport agent. The parent that signs must provide a valid government issued picture ID.
Under Age 14
Your passport will be valid for 5 years. You must provide:
- Two professional photographs taken specifically for a passport.
- A certified copy of your birth record (to be sent to the Passport Agency and returned with your passport).*
- Your Social Security number.
- Your parents' birth names, birth dates, birth places (State or Country). Both parents must sign in front of a passport agent. Both parents must provide valid government issued picture ID. Renewal Application
You can obtain a passport renewal application from a passport office or download the form from the U.S. Department of State www.travel.state.gov/ web page.
- Complete the form.
- Send in your old passport.
- Provide two current professional passport photos.
- The fee is $67 (valid for 10 years).
Passport Fees
Our passport agency will need two separate types of payment.
Payment 1: The Passport Fee and any Expedite Fee must be in the form of a check or money order, made payable to "U.S. Department of State."
Payment 2: The Execution Fee may be paid with cash or a check made payable to "Polk County Recorder."

Routine passports are taking approximately 8-10 weeks to be processed by the passport office.
Expedited travel within 2 weeks and 2-way express mail: is taking approximately 7-10 days - A copy of your itinerary is needed with this process and you must be leaving within two weeks.
Expedited with 2-way express mail: is taking approximately 3-4 weeks
To expedite your passport, there is an additional charge of $60.00. Ask us about the additional requirements for this service with or without express mail and/or the itinerary requirements.

Passport Photos
Two identical 2" x 2" photos are required in order to obtain a passport. These instant passport photos are available in Crookston at We Click, located at 110 South Main. Hours are Tuesday – Friday 10:30 – 4:30 or by appointment 218-281-2580. Fee is $12.00.
